Dear Parent/Guardian:
Burnham School will have In-Person registration on Wednesday, August 3, 2022, from 9:00 am to 1:00 pm, and Thursday, August 4, 2022, from 3:00 pm to 7:00 pm. at Burnham School, located at 13945 S. Greenbay Avenue, Burnham, IL.
All children must reside within District 154.5 boundaries to register for Burnham School. A list of requirements is below. Please note Proof of Residency documents must be current-no exceptions (ex., driver's license, utility bills, car insurance, voters’ registration, etc.). All required documentation must be submitted during registration.
Returning Student Requirement:
- Make sure you have up-to-date contact information on file
Kindergarten Student Requirement:
- Be five years of age on or before September 1st
- Official Birth Certificate (Official Seal)
- Illinois Physical Exam by October 1st
- Dental/Vision Exam
- Three current forms of Proof of Residency (driver's license, utility bill, voter registration card, car insurance/registration, real estate tax bill, rental lease, etc.)
New Student Requirements:
- Official Birth Certificate (Official Seal)
- Immunization record up to date according to state law
- Illinois Physical Exam by October 1st
- Three current forms of Proof of Residency (driver's license, utility bill, voter registration card, car insurance/registration, real estate tax bill, rental lease, etc.)
2nd Grade Student Requirement:
- Dental Exam
6th Grade Student Requirement:
- Illinois Physical Exam & Tdap & Meningococcal Vaccines
- Dental Exam
IMPORTANT:
ALL STUDENTS PARTICIPATING IN SPORTS MUST HAVE A CURRENT PHYSICAL EXAM.
