Health Documentation Requirements by Grade

 

This letter is intended to notify you of the basic health requirements for your child determined by their grade level. The standard immunizations required are not specifically listed, however, your child’s physician should know what immunizations are required based on the age/grade level of your child. Otherwise, immunization requirements for each grade can be found on the IDPH website.

 

Pre-K

  1. Immunization records

Kindergarten

Notes

 

* Vision and hearing screening will be provided by the school, dates are TBD.

** Dental exams and complete eye exams are required. The school may offer these services on-site (TBD at a later date) or you may take your child to a dentist/eye doctor of your choice.

 

  • All immunization and physical exam paperwork must be brought into the school prior to the first day of class. Your child may not attend school if these documents are not on file.
  • Proof of eye examination performed by an optometrist or medical doctor, if not offered by the school, must be provided to the school by October 15th.
  • Proof of dental examination, if not offered by the school, must be provided to the school by May 15th.
  • A concussion information form must be filled out for all students.
  • All students new to the district, regardless of grade level, will be required to have up-to-date immunization records, a physical exam, an eye exam, and vision and hearing screening.
  • Students in special education programs are required to have yearly vision and hearing screenings.
  • You are not required to bring new immunization records to the school each year. They are only required for Pre-K, Kindergarten, and 6th grade. If your child gets any new shots at any other time, you are encouraged to bring them in so we can update their records.
  • If your child has any medical condition (diabetes, asthma, allergies, seizures, etc) please notify the school nurse. You will be given forms that must be filled out.
  • All forms can be provided to you by the nurse, including sports physical forms which are only needed if your child will participate in an after-school sport.
  • Waivers are also available if any of the exams can not be completed for certain reasons.
