All children must reside within District 154.5 boundaries to register for Burnham School. A list of requirements is below. Please note Proof of Residency documents must be current-no exceptions (ex. drivers license, utility bills, car insurance, voters' registration, etc.). All required documentation must be submitted during registration along with the $30.00 Technology Fee (cash/money order).
Returning Student Requirement:
1. Make sure you have up to date contact information on file
2. 3 current forms of Proof of Residency (drivers license, utility bill, voter registration card, car insurance/registration, real estate tax bill, rental lease, etc.)
Kindergarten Student Requirement:
1. Be 5 years of age on or before September 1st
2. Official Birth Certificate (Official Seal)
3. Illinois Physical Exam by October 1st
4. Dental/Vision Exam
5. 3 current forms of Proof of Residency (drivers license, utility bill, voter registration card, car insurance/registration, real estate tax bill, rental lease, etc.)
New Student Requirements:
1. Official Birth Certificate (Official Seal)
2. Immunization record up to date according to state law
3. Illinois Physical Exam by October 1st
4. 3 current forms of Proof of Residency (drivers license, utility bill, voter registration card, car insurance/registration, real estate tax bill, rental lease, etc.)
2nd Grade Student Requirement:
1. Dental Exam
6th Grade Student Requirement:
1. Illinois Physical Exam & Tdap & Meningococcal Vaccines
